OC-076 LAPAROSCOPIC TAPP REPAIR FOR A LUMBAR HERNIA. STEP-BY-STEP SYSTEMATIZATION

Abstract Aim Lumbar hernias are a rare clinical entity and surgical treatment is constantly evolving changing. We propose systematization step-by-step guiding safe effective repair with similar conditions to the inguinal region. Material & Methods The patient lies down in dorsal decubitus position 45° elevation of hernia side. first port umbilicus access, accessory ports along middle line. Start dissection by marking anatomical limits that involve semilunar lines, epigastric vessels, spine, imaginary line between iliac crest 11 th rib. Releasing peritoneal flat until visualizing psoas iliacus quadratus muscles ribs, preserving avascular plane avoids nerves vascular damage. Hernia sac reduction. Placement polypropylene mesh (24×22 cm) covering all dissected areas. Mesh fixation using absorbable sutures. Closure peritoneum.Vacuum maneuver bulge compression. Results Systematic standardization was applied patients. mean operative time 90 min. Outpatient surgery achieved fast return usual activity. procedures were performed without intraoperative complications, mortality, or conversion open surgery. Conclusions guide for landmarks tips provides this entity. Finally, studies needed measure impact technique's implementation lumbar repair.
